Scope

Retention of Medical Records (Texas)

We retain adult patient records for at least seven (7) years from the date of last treatment. For minors, we retain records until the patient turns 21, or for seven (7) years from the date of last treatment, whichever is longer. We may keep records longer if required by law or due to ongoing matters.

We use limited analytics on general information pages and do not use tracking technologies on forms/pages where PHI is collected.

Payment Timing & Methods

Payment for services is due at the time of service unless prior arrangements are made. We accept cash, major credit/debit cards, and eligible HSA/FSA cards.

Insurance & Patient Responsibility

Insurance benefits vary by plan. You are responsible for copays, coinsurance, deductibles, and any non‑covered services. Refunds related to insurance will be processed after the insurer finalizes claims and all account balances are reconciled.

Cancellations & No‑Shows

Please notify us at least 24 hours in advance if you need to cancel or reschedule. Late cancellations and no‑shows may incur a fee that is not covered by insurance.

Services Already Rendered

Because medical services are delivered by licensed professionals, fees for services already rendered are generally non‑refundable.

  1. Prepaid Plans or Packages (if offered)
  2. You may request a prorated refund for unused sessions. Completed sessions are deducted at the standard per‑session rate (not the discounted package rate).
  3. An administrative fee of up to 10% may apply to cover processing costs where permitted by law.

Overpayments & Billing Corrections

Verified overpayments or billing errors will be credited to your account or refunded once all claims are finalized and any outstanding balances are resolved. Approved refunds are typically processed within 30 business days to the original form of payment when possible; otherwise, a check may be issued.

Products & Supplies (if applicable)

Unopened, unused medical supplies in original packaging may be returned within 14 days with proof of purchase, unless restricted by hygiene or regulatory rules. Custom or used items are not returnable.
